September 2026 results

Collected September 17. reported September 30. CO2 fell from 21 on July 30 to 18 mmol/L and is flagged Low (20–29). Creatinine is 1.36 mg/dL, flagged High (0.76–1.27). eGFR is 60 and has no lab flag (>59). HDL is 38 mg/dL, flagged Low (>39). AST decreased from 48 to 37 U/L and is within the 0–40 interval. LDL is 66, total cholesterol 113 and triglycerides 65 mg/dL. Hemoglobin rose from 16.2 to 17.1 g/dL and hematocrit from 48.2% to 50.5%. both remain within the printed intervals. PSA is 2.1 ng/mL, previously 1.6. Testosterone is 549 ng/dL by LC/MS, a different method from the earlier series. This panel did not measure ferritin, ApoB, sensitive estradiol or free testosterone, so their displayed values are still dated July 30.

Action Items
  • 1. Iron — recorded instruction remains off iron
According to the July record, the March start-iron order was completed with a short Ferrex-type course, followed by a multivitamin with iron and then a plain multivitamin without iron. July ferritin was 63 and iron saturation 39%. The recorded instruction was: do not restart iron. monitor at routine panels. September did not measure iron stores, so there is no new ferritin result and no revised iron instruction. Reviewed by Tess (Codex).
  • 2. Pravastatin and Ezetimibe — recorded July regimen
The March restart-statin order was recorded as completed by July 30: Pravastatin 20 mg plus Ezetimibe 10 mg. July ApoB was 61 mg/dL and LDL 71. September LDL is 66. ApoB was not remeasured. The July guidance was to continue the combination and re-trend at routine panels. These results do not verify current adherence or authorize a new medication change. Reviewed by Tess (Codex).
  • 3. FIT — completed July 24
The FIT collected July 24 and reported July 28 was Negative, completing the recorded screening order. It is a single point-in-time result. September added no new FIT, and the July result does not establish the cause of the earlier ferritin decline. Reviewed by Tess (Codex).
  • 4. Lipid recheck — September lipids, July ApoB
The post-statin lipid and ApoB recheck was completed July 30: ApoB 61 mg/dL, LDL 71, total cholesterol 112 and triglycerides 62. September adds LDL 66, total cholesterol 113, triglycerides 65 and HDL 38, which is flagged Low. The ApoB value is still the July result. The recorded July instruction was to continue therapy and re-trend at routine panels. Reviewed by Tess (Codex).
  • 5. Ferritin recheck — last completed July 30
July ferritin was 63 ng/mL, up from 12 in March, and the July recheck was marked complete. The prior record called for routine monitoring, with blood donations paused and daily aspirin recorded. September did not measure ferritin, so the latest result and completion date are still July 30. Reviewed by Tess (Codex).
  • 6. Estradiol and liver results — review status
The July record lists DIM 200 mg plus Calcium D-Glucarate 500 mg. It calls for a review of estrogen management after sensitive estradiol rose from 30.0 in March to 38.2 pg/mL in July (8–35). September did not measure estradiol. AST fell from 48 in July to 37 U/L in September, within 0–40. ALT is 33. These results do not establish why AST was previously high or show a need to change the recorded regimen. September CO2 is newly low at 18 mmol/L (20–29) and still needs clinician review. Reviewed by Tess (Codex).
  • 7. TRT recheck — total and CBC reported; free T missing
Decision recorded August 7: reduce TRT from 168 to 140 mg/week, effective August 8 (10 units/day from a 200 mg/mL cypionate vial). The requested recheck at 6–8 weeks was total and free testosterone plus a CBC, drawn before that day's injection and after 48 hours without hard resistance training. September reports total testosterone 549 ng/dL by LC/MS, hemoglobin 17.1 g/dL and hematocrit 50.5%, with no free testosterone. July hematocrit was 48.2%. Both September CBC values are within the printed intervals. The recheck as ordered is still incomplete, and because the assay changed, the difference in total testosterone cannot be attributed to the dose reduction. Injection timing and training conditions are unconfirmed. Keep the recorded dose decision dated August 7. any further change needs clinician review. Reviewed by Tess (Codex).

Interpretation Context

Creatinine, eGFR and CO2: Between July 30 and September 17, creatinine went from 1.39 to 1.36 mg/dL and eGFR from 58 to 60. September creatinine is flagged High. eGFR is unflagged. BUN is 17 mg/dL and the BUN/creatinine ratio is 13. CO2 is 18 mmol/L, down from 21 and flagged Low. These results do not show why creatinine or CO2 changed, and the previously recorded creatine use does not prove a cause. The new low CO2 result still needs clinician review. Reviewed by Tess (Codex).

Testosterone assay and recheck status: September total testosterone is 549 ng/dL by LC/MS at Esoterix (264–916), unflagged. In July, total testosterone was 1351 and free testosterone 40.4 on the earlier assay. Because the methods differ, the change from 1351 to 549 cannot be read directly as a response to the dose change. The August 7 decision reduced TRT from 168 to 140 mg/week, effective August 8. September includes total testosterone and a CBC but no free testosterone, so the requested total-plus-free recheck is incomplete. The report does not establish injection timing or training conditions. Reviewed by Tess (Codex).

Earlier testosterone results: The historical series includes 448 ng/dL in August 2025, 1165 in March 2026 and 1351 in July 2026. These values alone do not show missed doses or explain the variation. The September result was measured by LC/MS at Esoterix and is displayed separately from the earlier-assay series. Reviewed by Tess (Codex).

Iron history — last iron studies July 30: From March to July, ferritin rose from 12 to 63 ng/mL, iron saturation from 20% to 39%, hemoglobin from 13.8 to 16.2 g/dL, MCH from 25.0 to 31.3 and MCHC from 30.8 to 33.6, while RDW fell from 16.1% to 14.9%. In September, hemoglobin is 17.1, MCH 32.9, MCHC 33.9 and RDW 13.0%, all within the current lab intervals. September did not include ferritin or iron saturation, so it gives no new assessment of iron stores. The recorded July instruction was not to restart iron. Reviewed by Tess (Codex).

FIT history — July 24, 2026: The Labcorp ColoFIT, collected July 24 and reported July 28, was Negative (reference Negative). No occult blood was detected in that sample. This is the July screening result, not a new September test, and it does not establish the cause of the earlier low ferritin. Reviewed by Tess (Codex).

Urine results — September 17, 2026: Specific gravity 1.008 (1.005–1.030). pH 7.0 (5.0–7.5). yellow and clear. WBC esterase, protein, glucose, ketones, occult blood, bilirubin and nitrite: Negative. Urobilinogen 0.2 mg/dL (0.2–1.0). Microscopy: WBC none seen /hpf (0–5). RBC none seen /hpf (0–2). epithelial cells none seen /hpf (0–10). casts none seen /lpf. bacteria none seen. Urine creatinine 46.5 mg/dL (Not Established). albumin <3.0 ug/mL (Not Established). albumin/creatinine ratio <6 mg/g creat (0–29). The less-than values are laboratory reporting limits, not exact measurements. Reviewed by Tess (Codex).
